Leave of Absence, Pro Forma, Withdrawal from the University

Leave of Absence for PhD Students
Students who wish to apply for a leave of absence should contact humdos@uchicago.edu. The University policy can be found in the Student Manual.

Leave of Absence for Master's Students
Students in master’s programs may interrupt their program of study and request to take a leave of absence for a maximum of one year (four consecutive quarters, including summer quarter). Leave of Absence Status carries no tuition charges and confers no privileges such as registering for courses, accessing the libraries, borrowing money, deferring loans, access to health insurance or the Student Health and Counseling Service unless the student is on a medical or parental leave of absence.  Access to University e-mail accounts is maintained in Leave of Absence Status. Students who do not return from the leave of absence in the quarter following the approved leave will be withdrawn from the program.  They must petition to resume studies, subject to the approval of the Director of the master’s program and the Dean of Students. The Division will not consider petitions more than five years beyond matriculation.
 
Pro Forma
Pro Forma registration is a limited privilege (maximum six quarters per student) designed to provide full-time status to doctoral students who must move away from the Chicago area, usually abroad (but otherwise a substantial distance) in order to conduct preparatory field research, archival research, or data collection required for completion of the dissertation. Students who are engaged in formal programs of intensive language training instruction abroad, as needed for completing their doctoral programs, are also eligible. Students apply for Pro Forma status by completing an application form. After the for has been signed by the student's department chair, indicating recommendation for approval, it should be submitted to the Dean of Students' office for consideration. Students applying for Pro Forma who have received an external fellowship or grant (such as a Fulbright) should include a copy of the fellowship award letter with the application.
 
Pro Forma is granted for up to three quarters at a time. Students who need to extend their Pro Forma status complete a new application form and a one- to two-page statement addressed to the Dean of Students that describes the work accomplished toward the degree in the past year and indicates why additional time is needed in Pro Forma status to complete research in the coming academic year.
 
Withdrawal from the University
Humanities students who wish to withdraw should complete a Withdrawal Form, available in Walker 111. All withdrawals are assumed to be permanent. To resume study after such withdrawal requires reapplication to the degree program. Further information is available in the Student Manual.
