Change of Address:If you have moved, in addition to informing the Registrar and the Regenstein Library, please inform the department by filling out and returning the Change of Address form (word/pdf) to the NELC office.
Change of Name:
To change your name with the university you must fill out the official University of Chicago Change of Name form, which can be found here. Please bring a copy of this form to the NELC office as well so that we can update our records.
