The Dean of Students office does not print individual registration forms for students in Advanced (typically years 5-12) and Extended Residence (years 13+). Rather, departments provide the Dean of Students office with a list of students approved for quarterly registration. Students should check with their department coordinators for information on quarterly department registration ("check-in") procedures. Students in Advanced Residence (typically years 5-12) who wish to enroll in a course or language exam should fill out a blank registration form, have the form signed by an adviser (and the course instructor if applicable), and take the form to Walker 111. Students in Extended Residence (years 13+) are not eligible to enroll in courses.

Students in Advanced (typically years 5-12) and Extended Residence (years 13+) remain responsible for clearing all restrictions in order to regularize their registration by the end of the registration period. Failure to do so will result in late fees, and eventual administrative withdrawal from the University (consult General Information on Registration). The Student Manual of University Policies and Regulations provides detailed information on the University of Chicago's policy of continuous registration for doctoral students.
